Declaration I hereby declare that this dissertation is the result of my own work and includes nothing which is the outcome of work done in collaboration except where is specifically indicated in the text and bibliography and that my thesis is not substantially the same as any that I have submitted for a degree of diploma or other qualification at any other University. In addition, this dissertation falls within the word limit of exclusive of tables, footnotes, bibliography, and appendices, as set by the University College London. Abstract Total hip arthroplasty is a well established treatment modality for the diseased hip. The number implanted rises annually on a global scale which is mirrored by increasing indications. After aseptic loosening and infection, periprosthetic fracture remains one of the commonest complications of this otherwise successful surgery. Management is geared towards restoring function through fixation of the fracture. The general aim of this thesis is to validate the classification of periprosthetic fractures of the femur around total hip arthroplasty, provide evidence towards the outcomes of methods of fixation of these fractures, and present supplementary biomechanical data regarding fixation and implant stress. It is hypothesised that the Vancouver classification will be a reliable and reproducible system to use, that strut grafts, cables and long-stemmed implants will improve function and outcome when used to manage these injuries, and that biomechanical models will provide evidence on why the use of the implants is successful.

A biomechanical evaluation of the interprosthetic distance as a risk factor for periprosthetic fractures of the femur: does the gap distance matter?
Introduction The treatment of femoral fractures between a proximal and distal prosthetic stem is expected to become a major challenge because of the increasing life expectancy and the rising prevalence of osteoporotic fractures, Total Hip (THA) and Total Knee Arthroplasties (TKA). Although orthopedic surgeons intuitively consider a decreased interprosthetic gap distance as an important risk fac...
متن کاملTreatment of distal femur fractures with locking plates: Comparison of periprosthetic fractures above total knee arthroplasty and non-periprosthetic fractures.
The purpose of the present study was to compare the results and plate fit of periprosthetic and non-periprosthetic distal femur fractures fixed with locking plates. Twenty-one periprosthetic fractures above a TKA and 27 non-periprosthetic fractures were retrospectively reviewed. The primary healing rate, bone union time, clinical and radiographic results, complications, and additional surgeries...
متن کاملPeriprosthetic Fractures of the Femur after Hip and Knee Replacement
Objective: To present the clinical and radiological results of treatment of periprosthetic fractures of the femur after hip and knee replacements. Subjects and Methods: Thirty-four patients (8 males and 26 females) with 34 fractures of the femur complicating hip and knee replacements are the subjects of this report. In 21 cases, the fracture affected the femur after hip replacement, and in 13 c...
متن کاملLonger Length of Stay Increases 1-year Readmission Rate in Patients Undergoing Hip Fracture Surgery
Background: Proximal femur fractures are prevalent among the elderly and associated with substantial morbidity, mortality, and early readmission. Early readmission is gaining popularity as a measure of quality of hospital care and canlower reimbursement. A better understanding of the patient and treatment characteristics associated with readmissionmay help inform program improvement initiatives....
متن کاملMortality and Financial Burden of Periprosthetic Fractures of the Femur
OBJECTIVE This study examines patient factors to identify risks of 12-month mortality following periprosthetic femur fractures. Hospital charges were analyzed to quantify the financial burden for treatment modalities. METHODS Data were retrospectively analyzed from a prospective database at a university hospital setting. One-hundred and thirteen patients with a periprosthetic fracture of the ...
